WHAT IS THIS FORM FOR?
You can complete this form if the deceased left a Will. However, if the Will has been probated or letters of
administration or an election to administer have been granted by the High Court – you must complete Form21.
This certification should be filed with an application for succession (when grant of administration held) on form21,
and replaces Schedule 1 of that form.
Applying for succession
To apply for succession, you need to file a completed
application form with an MLC office by either visiting an MLC
office, attending one of our offsite clinics, or sending it by
post.
